To insure all patients are thoroughly educated, our office uses digital radiographes and an intra-oral camera.  Dr. Porterfield performs dental hygiene cleanings for patients with and without periodontal disease.  For patients with periodontal disease our office is a premier provider for the revolutionary PerioProtect system.  Our office performs all general dentistry  services including: crowns, bridges, full dentures, partial dentures, including metal free, flexible partials, composite fillings, root canal/endodontic treatment, extractions, minor surgeries, and teeth whitening.  Our ultimate goal is to provide you the best care possible.

You can fill it out on your own time and bring it with you to your appointment. If you forget your form or choose not to fill it out before your appointment, please arrive 15 minutes early to complete the form in our office.
